Tarek Shahbandar, MD

Dr. Shahbandar is a fellowship-trained specialist in Interventional Pain Medicine and is board certified in Anesthesiology and Pain Medicine. He was born in Michigan and raised in Wisconsin.  He attended the Medical College of Wisconsin and then completed residency and fellowship at the Cleveland Clinic Foundation. At the Cleveland Clinic, he was privileged to train under some of the world’s pioneers in Pain Medicine.  Since 2005, Dr. Shahbandar has practiced Interventional Pain Medicine.  His experiences span both private practice and academic medicine, including three years training physicians at the University of Illinois at Chicago. When practicing interventional pain medicine, Dr. Shahbandar has been well known for his kind bedside manner and excellent procedural skills. One of his goals when treating patients is to alleviate pain while minimizing the use of controlled substances. His specific fields of interest include complex regional pain syndrome and spine.  He founded Pain Physicians of Indiana in 2011.


Arif Hussain, DO

Dr. Hussain is a fellowship-trained interventional Spine and Musculoskeletal specialist. He was born and raised in the western suburbs of Illinois. He completed his medical education at the Chicago College of Osteopathic Medicine, where he learned the importance of comprehensive medical care for the mind, body, and spirit. He then went on to specialize in Physical Medicine & Rehabilitation at the prestigious Johns Hopkins University School of Medicine. Under world-renowned mentors, he developed skills in musculoskeletal medicine, interventional spine, and multidisciplinary pain management. He has successfully completed the specialty board examinations and is board-certified by the American Board of Physical Medicine & Rehabilitation. He went on to complete an ACGME-accredited, Interventional Pain Medicine fellowship at the University of Michigan in Ann Arbor where he learned advanced techniques for treating pain, such as neuraxial procedures and neuromodulation. Dr. Hussain is excited to provide his medical services for the greater community of the Northwest Indiana. When he isn’t practicing medicine, Dr. Hussain spends his time with his wife and kids and enjoys special outings to the numerous sporting and cultural events in the Chicagoland area.


Alexis Alvarado

Alexis Alvarado, Physician Assistant joined Pain Physicians of Indiana in June of 2014. She attended Benedictine University in Lisle, Illinois, where she earned a Bachelors of Science Degree. Alexis went on to graduate from the Physician Assistant program at Midwestern University in Downer’s Grove, Illinois and is board certified by the National Commission on Certification of Physician Assistants. Alexis is a strong patient advocate and her main concern is reducing pain and improving the overall quality of life of her patients. Alexis is grateful to have the opportunity to serve the greater Northwest Indiana area.


Angie Zielinski

Angie Zielinski, NP is a Board Certified Family Nurse Practitioner with advanced pain management training at Beth Israel in New York. She graduated from Valparaiso University with a Master of Science in Nursing (MSN) degree and completed a Post-Master Family Nurse Practitioner Program. Additionally, she is an adult clinical specialist (CNS). Ms. Zielinski is the recipient of numerous awards and honors including Graduate of Highest Distinction from Valparaiso University, the Nightingale Society, Academic Honors Award Member, and National Dean’s List, Academic Honor. She is affiliated with professional organizations in her specialty and professional fields. Her areas of clinical interest include Chronic Pain, Medical Management, Acute/Chronic Spine Pain, and Psychosocial components of acute/chronic pain. As a pain management nurse practitioner, Angie Zielinski’s goal is to help patients meet their potential when dealing with acute or chronic pain. She strives to treat each patient with compassion and work with each patient to define mutual goals during treatment. Her ultimate goal is to help patients manage their pain so that they can maintain their personal and professional activities as efficiently as possible.
